DOCTOR REFERRAL

When it comes to managing chronic conditions, seeking the guidance and support of healthcare professionals is crucial. If you’re looking for a comprehensive approach that combines exercise, education, and community support, consider asking your doctor for a referral to the YMCA. The YMCA is more than just a gym; we offers a range of programs and resources specifically designed to help individuals with chronic conditions. With the guidance of trained professionals, you can participate in exercise classes tailored to your needs and abilities, such as low-impact activities or water aerobics. Additionally, the YMCA provides educational workshops to help you better understand and manage your condition. By joining the YMCA community, you’ll also have the opportunity to connect with others facing similar challenges, providing a support network that can be invaluable on your journey towards improved health. Don’t hesitate to talk to your doctor about this referral, as it can be a significant step towards taking control of your chronic conditions and living a healthier, more fulfilling life.

If you are a referring Dr, you can be confident your patient will be enrolled in specialized programs designed specifically for individuals with chronic conditions, such as diabetes, heart disease, or cancer. These programs typically involve a personalized exercise plan, nutritional guidance, and support from trained professionals.

Living with a chronic condition can be challenging, but it doesn’t mean that making positive lifestyle changes is impossible. In fact, it’s even more crucial for individuals with chronic conditions to prioritize their health and well-being. If you find yourself in need of some extra motivation, ask us about our Medical Memberships.

Individuals are much more likely to participate in a program when they are directly referred to the YMCA, allowing YMCA  staff to engage directly with the referred individual and connect them to the most appropriate program option.

If you would like to refer your patient for participation in any of our evidence-based health intervention programs or more general health behavior change support, please feel free to send the referral to us via the secure email: Erin Widener

Our Y Healthy Community Director is happy to visit your clinic and provide presentations and/or written materials to increase provider and patient awareness of YMCA program goals, how to refer, and what to expect. We also offer leave-behind resources for both care teams and patients, including brochure, posters, and referral cards. These materials can be co-branded if desired. Please contact Erin Widener for more information.

Thank you for entrusting the YMCA with supporting individuals in your care! It is a responsibility we do not take lightly and a true privilege.
